1) Install the module as any other module (I think you already did that)

2) Drop the module inside any page you like (Give access to only admin users)

3) Go inside the INSTALL HELPER option inside the module

4) Enable the HACK

5) Go to the GENERAL SETTINGS option (In the main menu of the module)

6) Enable what you want to localize and press update

Note: You must use the skin object that comes with the module. There is a sample skin in my website. also, you can use the locale selector that comes as a module the same way as the skin object.

Sorry. I work alone on my free time. But you can always buy the sourcecode and try to do it yourself

About your points:

1) There is a sample skin on my site taht you can use as a base for your own skins. Including  the locale selector is as simple as including the [DSLOCALESELECTOR] tag. The parameters can then be specified in the skin.xml file (Look at the sample). Also, if you drop a DSLocaleselector module in your page, you can test the configuration and ask the module to generate the XML content for the skin object

2) I would like to know about

3) I tested the module in most know browsers and no bugs were found. The module works behind DNN, so it can work where DNN can work. Please comment on this.
